Follow these steps to get information about selecting your Primary Care Physician with AllCare through Secure Horizons:

If you are already enrolled in Secure Horizons, you may be able to transfer to AllCare and keep your current Primary Care Physician by calling Secure Horizons at (800) 228-2144, TTY/TDHI #(800) 685-9355. Hours of operation are Monday through Friday, 7:00 a.m. to 9:00 p.m.

• Tell the representative that you wish to remain assigned to your current Primary Care Physician, and would like to choose AllCare as your Medical Group.

If you are Medicare eligible and are interested in learning more about Secure Horizons Medicare Advantage Plans, call Secure Horizons at (800) 385-5588, TDHI# (800) 387-1074. Hours of operation are Monday through Friday 8:00 a.m. to 5:00 p.m.

• Ask the Sales representative to tell you about the advantages of Secure Horizons.
• If you wish to select and/or change to Secure Horizons, tell the representative that you would like to choose AllCare as your Medical Group and keep your current Primary Care Physician.
